Welcome and Thank You!
Thank you for your decision to serve the Lord here at Selah Hills Church! The desire to serve is an evidence of the work of Christ within you. “Where to serve,” becomes the big question. This ministry questionnaire has been designed to help us to get to know you better to assist you in determining your place in the Body of Christ.

Each of us has a unique blend of experience, talents, traits, gifts, and past experiences that God will use to accomplish His work. In 1 Corinthians 12, the Apostle Paul uses the “Body” as a metaphor to describe how the local church should function. Each one of us has a unique part of the Body of Christ, and just like our human bodies, each part has a specific purpose.

The motivation for our service should flow directly out of our love for the Lord Jesus Christ. As we are in love with Jesus and serving His people, there is a tremendous joy that we experience. Jesus said, the greatest in the kingdom is the servant of all. It is an honor and a privilege to be a servant of Jesus Christ and to serve His people.

Next Steps:
Following this application, you will receive an email to complete a background check (if you are under the age of 18, you can skip this step). This will come via email from "Checkr." Because many of our areas of ministry involve children, and other groups which require these background checks, we have found it necessary to have all ministry volunteers screened since so many times we have a need to call upon one area of ministry for help in another.

We also want to encourage you that just because you may have something in your background that preempts you legally from serving in an area of ministry, we feel that no one has sinned so greatly that the blood of Christ cannot make a place for you to serve in the Body of Christ. We may not be able to offer certain avenues of ministry because of legal prerequisites (Romans 13), but we can find a ministry area in which God will be glorified through the gifts and talents He has given you.

Trust God and He will prove faithful in placing you exactly into the area for which He designed you in the Body of Christ. Don't be afraid to try serving in different areas as you learn and grow in your callings. We are excited for you to complete the ministry questionnaire and find your calling to serve within the body of Christ here at Selah Hills Church!

Servant Requirements:

You must know Jesus Christ as your personal Lord and Savior.
You must have attended Selah Hills Church consistently for at least 2 months to serve on a "Serve Team" and consistency for at least 6 months to serve on a "Ministry Team."
You must agree with the Selah Hills Church Statement of Faith.
Following the completion of this application and background check, you must also have a discussion with a Ministry Leader before serving in any given position (you will be connected with a ministry director after completing these steps).

The following also applies for those serving in “positions of leadership”:

You must have walked with the Lord for at least 2 years and have been publicly baptized.
You must be willing to live a life that is “above reproach” and “give no appearance of evil” (1 Thessalonians 5:22). This includes drunkenness, sexual immorality, substance abuse, social media posting, coarse joking, etc.
The Ministry Leader is called to set the example in wisdom, right living, and consideration for others.

In consideration of my serving role with Selah Hills Church, its affiliates, and supporting organizations, hereinafter “Selah Hills Church” and recognizing the importance of my character being above reproach, I HEREBY CONSENT TO THE FOLLOWING:

● Complete in full a Selah Hills Consent and Waiver Form.
● Complete in full a Selah Hills Background Information Form. Please include your last known address if you haven’t lived in Texas for seven years. (If under 18, no background check is needed)
● Allowing Selah Hills to screen any other criminal history records as well as any other historical or background records pertaining to me.
● Allowing Selah Hills to verify the information and to contact any personal references listed on the Background Information Form.
● Make myself available for a personal interview with the Pastor (and/or Ministry Leader) of Selah Hills if needed.

I HEREBY WAIVE, RELEASE AND HOLD HARMLESS FROM LIABILITY all persons, organizations, and other entities which provide references or information to Selah Hills.

I HEREBY FURTHER WAIVE, RELEASE, AND HOLD HARMLESS FROM LIABILITY Selah Hills, it’s staff, employees, volunteers, and agents with regard to any decision that it makes on my application for involvement with Selah Hills based on the information I provide or that is obtained through the criminal history and background screening process.

I consent to a copy of this Consent and Waiver Form together with the Background Information Form being furnished to any reference that I have provided to Selah Hills and to any other person, organization, or entity that Selah Hills deems necessary in connection with its investigation of my background, character, or qualification.
